So I include awards that I got back in undergrad (I have been out of dental school for 3+ years), and organizations I am a part of (ie ADA, AGD). Should I include volunteer experience from undergrad as well.

I think you should include information from undergrad. Most of the extracurricular activities, work experience, etc. was a rehashing of my cv. But if you have an opportunity to give them the information, I see no harm in doing so.
If you've ever seen application files taken apart, photocopied and shared around a table, you'll understand why more information is NOT a bad thing. Good luck.
